You are hereby notified, pursuant to A.R.S. § 33-1366, that the leased premises have been damaged or destroyed by fire or casualty to an extent that enjoyment of the dwelling unit is substantially impaired.

NOTICE OF TENANT’S INTENT TO EFFECT REPAIRS
